Re-vamp of application forms

Hi PPH,

I am looking for a collaborative parter who can help me take my company application forms, and give them a visual refresh.

At my company we use basic pdf forms, that we'll fill out for a client, then send via docusign for signatures. Our current forms arent very visually appealing, let alone intuitive. Im hoping to have someone who can have some ideas about ways to clean up the formatting of the forms, streamline them as well hopefully.

The second aspect of this job is Im looking to have the new refreshed form visible online for folks to fill out by themselves. So, in the same vein as above, Im hoping to make the form more user friendly, and easier to navigate etc.

Hoping to have someone who will let me make a few rounds of revisions as well without being too annoyed with me.
