Questionnaire design

I need some new content/text/questions put in a layout i already have for another questionnaire. I got the original questionnaire (original questionnaire.pdf) and can provide the .indd if needed. The new questionnaire should be more or less the same design, but should integrate the different questionnaires below in the design. All text ist in german, but i will do the proofreading. I will need the source .indd file for future adjustments.
