Ordering System

We need an ordering system created something similar to the attached image:

1, create a form where we fill out all details for the delivery
2, after you order it goes to another page where it stores all details in a datebase and also prints off the order to fax to supplier
3, create another page where we can search for this order from the database for checking off the order, search has to be from any of the fields for instance by Reference number, By Delivery address, by date, etc etc or even a key word within the materials ordered.

order number needs to increase after every order so no duplicates are made
