Indesign branded questionnaire

I am looking for a branded Indesign questionnaire which will allow me to publish an interactive PDF - someone will ask a series of questions interview style and then populate the answers into the interactive pdf. there are 4 sections - 2 sections have about 20 questions and the other 2 sections have 6 or 7 question each. So I need the questions and then a space to type (or write if printed off) answers. I need a fast turnaround please. Sample attached
