I need a PDF timesheet formatting into Word

Design details: Dear All,

I have designed a timesheet for a client who now needs it in word format. There's no design required just the ability to recreate the design word for word, line by line in Microsoft Word. I basically need a Word version and exact replication of all elements.

It's important that we use the exact same layout, spacing, line widths etc. No deviations from the original will be accepted. The only thing that can change is the font. We will have to choose a font that is common to all PC/MAC users.

If you are a Word whiz this will be something that won't require too much thinking just a little time and patience.

I'm looking to place this with the successful candidate Monday 28th November with a turnaround time of 2-3 days. Revisions will be required if your version does not match the original in the areas highlighted above. Please only send a proposal if you are familiar with Word on a intermediate to high level.
