I need a business card image

I need the attached image centered at the top of a business card image, with the diameter of the black circle 41mm. None of the black circle should be in the bleed area, but the card background can be black. Other specs from the printer:

Size without bleed: 2" x 3.5"
Size with bleed: 2.125" x 3.625"
Resolution: 638 x 1088 pixels (300 dpi)

Please send the final image in a GIMP compatible file. Thanks!
