Editing and formatting 4 documents

I am a therapist and have created four documents that I would like to share with other therapists as templates, allowing them to simply add their own logo. Currently, these documents are in draft form and have poor formatting. I need someone to proofread the grammar, enhance their professional appearance (making sure they all look the same in terms of font, colour etc), and ensure the formatting is fixed so therapists can easily type into the designated fields of the assessments and reports without this changing and looking messy. Additionally, I've used 'X' as a placeholder for the client's name, which needs to be easily changeable.

Please provide examples of similar work you have complete in the past.

These need to be in word document.
