Design an Inlay Card - Foldable

I need a folding inlay card (this will fold like a book) with 4 faces. Face size in 80mm x 80mm (after folding). This card will fit inside the HKS Jewellery branded box (as shown in attached photos)

Face 1 : will have HKS Logo
Face 2 : jewellery care instructions
Face 3 : Jewellery care instruction
Face 4 : Discount Code for returning customers and facebook and instagram addresses

I have made a template to help with understanding the requirements. Please see attached images. This is really two faced card which would be folded from the middle
