Also there is a PDF attached as an example they want it to look like.
Fonts and Logo Assets will be provided separately.

There should be a black header with the logo added to the top of the form
CMYK codes are:
Gold: C:20 M:40 Y:50 K:10
Grey: C:0 M:0 Y:0 K:40
