Create a editable invoice and letterhead for my business

I need a professional to create an editable invoice and letterhead for my business. The invoice needs to be able to allow me to input data to the invoice without disturbing any mandatory information on the invoice and also should not disturb the layout and format of the invoice. I am attaching an invoice someone tried to do me but the invoice did not allow me to input data without causing issues, please view the attachments, the invoice needs to allow me to enter information with ease and without disturbing anything on the invoice.
I don't want to have to receive a document and find many issues with the job that I specifically made clear should not happen when I posted the job.
