Convert my word file into a prestation template booklet

I need a graphic designer who can take my old word file template and turn it into a modern professional better looking template.
Looking at something architectural, creative, something that's easy to read and understand.
All of the content in my word file needs to be added to the template. Just in a different look and feel.

The areas in yellow are areas I do change the majority of the time.
The template needs to be editable in all areas as scope of works can change.
I have access to all Microsoft office software and adobe indesign.

I want the template to be quick and easy to use. This is imperative.

This will be sent out to clients.
In the future the logo will be changing. That is another project that will be done separately.
