Build pdf infographics from csv data

We're looking for someone who can provide an editable pdf (possibly Indesign or Illustrator) that is populated by data from a CSV file.

The pdf will have 15-20 sections where text/numerical data will need to be updated from the csv.

If possible image variations could be made via the cvs - ie, if the data reads YES/NO a different graphic is used. We're not sure if this is possible or not?

The attached image is a good example of what we're looking for from a design. Any large numbers and some text areas should be updated via a csv.

Any advice on whether it's possible to automate multiple pdfs (each generated from it's own row in the csv) would be appreciated.
