1 word logo for a t-shirt print. Vector, AI, EPS or PDF file.

Hi,
I would like someone to copy a logo into a file suitable for it to be printed onto a t-shirt. I have attached a document with my requirements. Please don't hesitate to ask any questions. The design has to be an exact replica or something very similiar.

The design will be printed on 3 different sized t-shirts (Small, medium and large), so I need the design to fit proportionally on the different shirt sizes. This may require 3 different files! or the file to be flexible enough to change size.
