Invoice design

Attached is my rough mock-up of an invoice design that I would like created. It is also necessary that the invoice has specific input fields as this invoice is needed for only one purpose of payment.

I will need to be able to fill out the form & fields, and save and print the invoice to send to customers.

All information, artwork and logo is available.
Immediate start preferred.
