I need a logo, business card, email signature and flyers designed

I have a small start up and we are looking to develop our brand and customer face. As such I'm looking for a graphic designer to come up with brand templates to include

Company logo - must be in a standard image format to allow us to use in order documents and templates.
Business Card - front to be contact information, rear face to be company logo. We would like a version we can edit, if we change address or phone or as we take on new staff.
Email footer - contact information and company logo. We currently use gmail so it should be compatible and editable.
3 x A5 flyers, in order to promote the various services we can offer if clients. Ideally we want a standard company theme that varies slightly for each service area flyer, like a change of colour for instance.

In your proposal I would like to see;
A price per item required as well as a total fixed price
How many amendments we would get included in your price
Your turnaround time for each
If you have samples from similar jobs to demonstrate your quality that would be appreciated

All text to be provided to the successful candidate. For information we provide business consultancy, so our band needs to be professional and no nonsense, currently use a lot of blues in our materials so would be keen to keep . We've spent a lot of time making sure we offer something different, so our logo needs to express this and not be a repeat or standard. Our company name is LMS Consultants (subject to being registered shortly).
