Google review, raffle ticket draw and a loyalty scheme info

Hi, please look at Undercastlecottage.co.uk. As you can see, we offer a very high quality, selfcatering property.
We need the following paperwork made for our guests when they arrive at the cottage.
1 Google review and social media - hopefully they will fill it out when they are there
2 Loyalty scheme called Golden Carat Certificate with pony points - a discount towards the next booking
3 Enter into an annual raffle to win a stay at the cottage - up to 6 raffle tickets so that we can obtain 6 individual email addresses for marketing

I have some ideas but, I am not computer friendly. There must be consistency between all 3 things as they will be in the same envelope.
I like the idea of using the line drawing.
It would be good to discuss on the phone.
