Editable PDF

I have an editable PDF document, which for some reason will not print, it comes out of my printer as blank white paper only. I am using Adobe Reader.... is there a better programme to use?

I require this fixed and a couple of small design changes, to improve the look of the document. Such as the "payable to" box is a bit boring. What can you suggest as an improvement?
