Design Background - Headed Print A4

We need an A4 background and headed paper print, designed for a business.

Please see the screenshot (of which you will be provided print-ready files), then recreate, and adjust the design to suit.

1 - The background should be blue, with the text design all over

2 - There should be the company logo, top left

3 - There should be these company details at the bottom/footer:

5 - Note the font is 'Catamaran'

6 - The background should be faded out appropriately, as will be printed over etc...

7 - The logo and footer text should not be faded out.

8 - You must strictly keep to the company design and colours ONLY.

9 - The style is corporate and professional.

10) At the bottom, there should be a QR code, and under it, it will say:

"We value your feedback! Scan this QR code to leave a quick Google review."
