Typesetting for Amazon KDP - ePub, paperback and hardback

I have 62k word book to publish on KDP, currently in docx format. It's about 180 pages and with approx 20 b/w line drawings.

I need it laying up for Amazon KDP in the following formats:

* ePub (I will supply cover)
* soft and hard cover pdf

Amazon print options for pdf as follows:

* B/W interior with white paper
* Trim size 6x9 inches
* Bleed settings - Bleed

I need the soft and hard cover pdf laid up as in the sample below...
