I need an InDesign CS5.5 (Windows) file typeset

Design details: I have been trying to learn InDesign and use it to recreate a document that I need to substantially edit. Whilst I have been able to do most of the work in putting the document into InDesign CS5.5 (for Windows) , I am struggling with some of the typsetting throughout the document and recreating an application form.

I need someone to review a 48 page InDesign file that consists 95% of text and improve the typesetting, including making the application forms towards the back of the document look professional. I have attached a pdf of a document that is very similar in design and layout to the document I am working on (but it is NOT identical - a fair bit of the text has changed)
Ideas for the visual style: Supplied - this is typesetting/layout
Extra notes: I would have thought this is only 2 hours work, perhaps even only an hour, providing you are good at typesetting text based documents in InDesign. I need this done ideally within the next 24 hours. Happy to pay for 2 hours work, and but review after you've spent an hour and see whether it's good enough. need this back in as an InDesign 5.5 (windows file) - don't worry about the covers, or pictures/links - except for the application forms. This won't be a difficult task for the right hourly and I've got good seller feedback so far! :-)
