I need a book cover and layout designer.

Design details: Hi I have just completed a manuscript for a new book titled Title: Write Now / Subtitle: A quick and easy guide to write and publish your own book.

I am looking for a clear crisp professional cover and interior layout design with shelf appeal. I will also need to printed version, ebook, epub, mobi and all other versions as well. I need this design finished within the next two weeks is possible. The book is approximately 20k words. I not sure what trim size would be appropriate for this size book I will need your help deciding that.
