I need a 4-page welcome pack creating

Hi guys,

I have the wording written and the brand guidelines, I just need someone skilled to turn it into a good-looking welcome pack for my clients to attend my clinic.

I want a branded presentation folder, big enough to contain 4 A4 pieces of paper that explain to the client what the procedure is.

I have attached the 6 pages I want designed (page one & 6 is the presentation folder)
