Design book(let) and prepare print-ready pdf

I am planning to print a short history of the road I live in for very local distribution. I need someone who can produce an accessible and attractive print-ready pdf from between 8,000 and 10,000 words of plain text, three tables of information (the largest of which occupies just over two sides of A4 portrait @10pt) and up to ten maps and diagrams.

I would also like a straightforward cover design that incorporates a photograph.

I will need some advice on font and on general format (eg A4, A5 or other).

The material will be ready to go no later than 6 April and I would appreciate a turnaround of less that 10 working days.
