IT Project Manger - Service Transition experience Required

I’m currently managing the transition from project delivery to support for a SharePoint NHS project, and I’m looking for guidance on the required project templates and the information needed for a smooth handover.

If you know where I can purchase the templates or provide guidance/coaching, I’d be interested in learning more. As a Project Manager, I’m specifically looking for support from a senior PM who can assist with this transition.
