Create a List of Documents

Part 1 of the task is to write the index for a bundle of documents in a list format. Part 2 is to create that bundle of documents in 1 PDF document. I have attached the current draft disclosure list and the list of documents you sent me that I have formatted correctly. The list is to be written/entered on a prescribed form called an N265 template which will be provided. You will see that Form N265 is amendable to add documents descriptions and dates to each row.

I will provide the information that has to go onto the N265 template.

I need this task done in one day.
