Create an interactive PDF form with logo etc

I'd like one document made from elements of both those attached with a header page where the Questions and Learning Outcomes (only those that match the numbers at the end of each question) can't be changed but individuals can write their names etc and answers into the document, and can then cross reference page numbers and digitally initial boxes. This is the first time I've used this site and am not sure how much detail to give here! I'm hoping explain better as needed :) I also appreciate I may not have picked the correct category but knew I had to start somewhere. I will have more of these to create in time.
