Spreadsheet Creation

Hi there,
I need somebody who can create a GOOGLE SHEETS spreadsheet for me that has a column which has different drop down boxes to calculate different % off a net total.
So, for example:
Net amount received £500.
Then the following boxes
Own Business 100%
Own Business 2 80%
Own Business 3 75%
Own Business 4 92.5%
Introduced Business 1 65%
Introduced Business 2 50%
Bubble Lead 1 65%
Bubble Lead 2 50%
The columns need to include:
Adviser Name
Client Name
Lender/Provider
Account Number
OPP ID
Policy Type
Net Banked
Payable to you
Then a sub total
Then deductions boxes before the total for:
Revolution Fees
ID Checks
Fees C/F
Then a total box
