Small typing task

Small typing task as per screen shots - 3 pages of legal form plus 1 page of text. Text and format need to be re-created precisely as given. The 3 form pages are mainly blank / white space approx (20% 10% & 33% text respectively).
NB Pages will be printed for physical signature after the forms are completed.

Please deliver:
1) Draft copy to be submitted as pdf for review - the format is the primary important factor.
2) A PDF and microsoft word version to be submitted after correction of any mark ups for payment.
