Simple Excel data entry

On the page attached there are 3 pairs of code/quantity ("codigo/QTD). [Text in "descricao" isn't relevant].
I need them on an Excel file with 4 columns:
1="Code"
2="Quantity"
3="Page Number" (in this case/sample: "5")
4="Column Number" (1, 2 or 3) [this 4th column is just to identify the localisation of the information more easily].

Notes: a) In case you're not entirely sure about a number just simply highlight that cell, please; b) On the quantity cell you just have to put the sum total (1+1+1="3").
I have an equivalent of 20 pages like the one attached to be put on an Excel file.
Please, provide a budget and delivery time (ASAP).
