I need a PDF document to be edited.

I have a PDF questionnaire that I require to be simply edited. I need the questionnaire to have the company specific infomation removed including logos, loaction and names. It also needs to be slightly reformatted so that the multiple answers to questions can all be seen along with which answer has been selected, this wiill require space to be made for the extra answers within the document. This should be a straight forward job for someone used to formatting pdf documents. I have hand marked the changes on the document and I'm happy to scan it to be viewed before acceptance of the job.
