I need a PDF converting into excel and then editing,formatting

Description of excel work: I have a PDF document of standard phrases that i need converting into excel format.
Once converted i will need it formatting so it can be used by a software company to import into their bespoke software that i will use on a IPad.
below is extract of typical phrases. The pdf document is 89 pages

Extra notes: Please see an excel example document for your phrases which I have enclosed. On the responses tab you will see four columns.

Column A- (REQUIRED) is a drop down list of the bespoke sections of the HomeBuyer Report. You must pick the exact section where you would like the phrase to be available. In the example enclosed this phrase would be available in the Section: E Outside the property Sub Section E1 Chimney Stacks

Column B- (REQUIRED) is the response title. These are bespoke to each phrase. They should be short and sweet as on the iPad you can only see a handful of characters.

Column C- (NOT Required) is Response Path. This allows you to create a folder to group like items together. In the example enclosed you might want separate folders for flues, stacks and pots so that you do not have to scroll through a long list of options.

Column D- (REQUIRED) is the Response Text. This is where you include your phrase. When adding in your phrases we can even build in what we call tokens. These are variables that light up green on the iPad and trigger the surveyor to do something such as pick an option or a command like Describe.

When sending your phrases if you have options you would like included please separate them with backlashes /
Example: The weather at the time of inspection was hot/cold/wet/dry/warm/sunny/raining. Here we would know to make those options active so that while you are out on site you can pick the relevant options depending on the day.
If you do not have any options but you want a token to trigger an action you can type in DESCRIBE
Example: The weather at the time of inspection was DESCRIBE.
