Formatting a document from text to HTML using supplied guidelines

I have a document, approx. 5000 words, which needs to be converted from text into a .htm document with HTML tags for formatting.

I have specific rules to apply to this document. For example, a particular HTML header is required; tags indicating divisions for code; image tags required to be applied to the document; list items to be bullet-pointed.

The work should take someone about an hour in total. Documents and guidelines will be supplied on acceptance. Questions welcome. Freelancer should be familiar with HTML tags and how they work.

Items to be returned should be a single document with all tags in place as per supplied guidelines, able to be rendered in-browser.
