Fire Risk Assessment word document made in to an editable PDF

I have a Fire Risk Assessment in a word document that I need made into an editable PDF with "drop down" menus for date input and also "tick" boxes for "Yes" "No"

The document is already branded and 90% complete I just need someone with the technical ability to make it into a functioning document with free txt boxes and the above requirementents.

I also need the "Action Plan" page to have drop-down menus for the "Priority" and "Timescale" sections the rest of that section then needs to be able to have free txt writing into it.
