Excel formatting/multi level drop down lists required

Attached is a template which I want to use to migrate data. I want someone to format the cells (as indicated by comments across row 1 of the attached document) to ensure the template integrity remains good when being edited by parties entering their data. It's fairly simple and basic and just need the following changes applied
- Mandatory cells (and info on how to apply to any further cells if needed)
-Uppercase formatting
-Lock rows/columns from being edited
-Multi level dependant drop down lists
