Data Entry of Approx 400 items on spreadsheet

I need someone to take 350 items on a spreadsheet and separate the information onto 4 separate columns. This is mainly through cut and paste. Additional information is to be taken from folders and subfolders of invoices / receipts and timesheets from a google docs folder.

Please study the word document with precise instructions before tendering. Feel free to ask any questions, especially if anything does not make sense. An example of a few lines completed would be very helpful.
