Data Entry

I have an excel spreadsheet with First names and Surnames and the Email Formats for work emails. All I need is someone to go through and follow the email formats to create the email addresses for the contacts. There just over 5K in the excel document. It is a very simple and fairly quick job but i need to finish off a current project and time is running out for me to do everything. This is how the spreadsheet will look and all i require is someone to do the same as I have on the first one "Stephen Archer" :
