Covert PDF forms to Docusign

I have some PDF/Word document forms that are used for on-boarding new drivers in our Taxi business. There are combinations of data input types (names/address etc) plus also tick boxes and signatures etc. I've attached 1x PDF version as an example.

We have a docusign account that our staff currently utilise, but we are not experts.

We would also like to be able to send this out to the client by SMS. Currenlty we use email. We are unsure is sending an SMS link is possible.
