Convert PDF file to excel with proof read ( MUST BE ACCURATE )

Description of excel work: I need an index from an aircraft parts manual (PDF) converted to a excel spread sheet. (.xls extension preferred) The use of PDF converting to xls software is required. After conversion the xls file must be proof read and corrected to be EXTREAMLY ACURATE.

Included is the actual pdf file that I need converted and an excel file containing the 1st page results of the conversion process. I need all pages in one single excel file and in the format shown in the sample excel file.
Design and target audience: PDF conversion software and excel software
Extra notes: We are looking for someone that is very skilled in the use of pdf conversion software and provides extremely accurate results.

The index we need converted is one of many indexes that we will be converting to complete our database project. We need someone that can produce quick and accurate work.
