AFA has evolved into a community level program that emphasizes and promotes the multi-dimensional approach of awareness, counseling, support and surveillance to improve drug abuse and HIV/AIDS prevention and control outcomes in Parsa district. The project not only delivers services that disseminate information and behavior change expertise to Injecting Drug abusers (IDUs) and IDUs living with HIV/AIDS (ILWHA) but also encourages parents and key influencers of youth to continue their healthy actions to prevent future drug problems.
Clients are encouraged to enroll in AFAs self-help drug rehabilitation treatment program and become an AFA role model in the community. So far, there are 14 role models and they are presently working for AFA. There are currently 10 volunteer staff (mainly recovered clients) and 55 field volunteers working under the AFA program.
At present, we deliver the following services:
I) Fellowship Counseling (FC),Self-Help Rehabilitaion and Referral for Treatment
As the provision for counseling, fellowship and support to drug users remain narrow and continue to be inadequate in Birganj and its surrounding districts, AFA is providing these services to drug users, mainly IDUs and ILWHAs. The AFA center offers free fellowship-counseling to drug users that guides them through a series of self-help therapies and then refers to a drug treatment program, if needed. AFA self-help and fellowship programs are based on a simple set of ideas and steps. They are suggestions rather than a set of rules that encourage drug abusers to find their own personal recovery, the one that is the most meaningful and practical.
We connect suffering addicts to recovered clients/role models who lead newcomers through a personalized one-on-one study that helps determine whether a drug detoxification program is needed. Recovered clients/role models, who work as Fellowship Counselors, provide one to one counseling to ensure maximum moral support and a comfortable environment. Clients are then referred to one of the rehabilitation centers in Hetauda or in Kathmandu. After recovery they join the AFA recovery group and enroll in the day-care program for a month at the center. So far 44 of our clients have undergone AFAs self-help drug rehab therapy out of which 9 are working as FCs at the AFA center.Altogether, presently there are 550 drug users signed up with the center.
In the coming days by increasing our financial resources, we anticipate to provide a professional counseling program and inclusive support to IDUs/ILWHA in Birganj that addresses the deficits of comprehensive rehabilitation and support.
II) Basic Medical Care
Our AFA Community Clinic primarily focuses on the initial management of local and systemic complications of drug use and then on referral to appropriate drug treatment programs. The primary mission of the clinic is to provide primary health care to drug users and assist them in receiving appropriate medical care without prejudice and free of charge. In this endeavor, Sano Paila has coordinated with the District Health Office, Parsa and Narayani Sub-regional government hospital, Birganj to ensure that AFA clients receive further consultations and treatment for infectious disease and other medical support, including care for HIV infection and AIDS.
We aim to enhance our medical services by increasing health-manpower and upgrading the clinic and treating medical ailments and complications due to injecting drug abuse, including various opportunistic infections. We further seek to provide primary emergency services and field-based services that will include free mobile-health camps for IDUs/ILWHA.
III) Harm reduction and Peer led intervention programs
AFA harm reduction programs deliver outreach services to IDUs/ILWHA in their own community and operate in a variety of settings, including storefronts, sidewalk tables, health clinics, and places where IDUs gather. The key activities under this program are:
a. Education about safe injecting and prevention of HIV/AIDS
b. Access to sterile syringes and promoting use of sterile equipment (Syringe Exchange Program)
c. AFA Peer Led Intervention Program
24 trained Peer Volunteers (users) in 19 wards of Birganj Sub-Metropolitan.
d. AFA Volunteer Training and Recruitment
38 AFA Volunteers in 19 wards of Birganj Sub-Metropolitan.
IV) Enhancing community awareness, participation and advocacy
AFA has organized and hosted various interaction programs to build partnership with different sectors of the community, especially risk groups and policy makers, and has encouraged discussions on issues of drug abuse and IDU-related HIV infection. Our role models and recovered clients share their personal experience and openly interact with participants. Our Media and Information team has produced a documentary -“Fortunate Drug users?”, which is a unique presentation covering real stories that reflect the injecting drug abuse scenario in Birganj and its cross border area/s and also encourages viewers to take a little step in building a drug-free community.

b. Awareness Campaigns and Mass Rally
1. World AIDS Day Rally • December 1, 2007
Around 300-350 people participated in the rally and displayed placards and banners with the World AIDS Day slogan and 200 t-shirts with AFA slogan were distributed.
A group of 11 children displaying the banner “Stop AIDS, Keep the Promise: Leadership” and leading the rally was a major attraction for the community residents.
2. International Day against Drug Abuse and Illicit Trafficking-26 June, 2008
A. Walk Rally Campaigns
2008- The attraction of this rally was an 8 year old boy displaying a placard that said drugs isnt life, its death” and leading a mass of 200-250 people. Diverse groups staged placards in different languages and a lakhe (cultural) dance was performed. Around 150 t-shirts with the AFA slogan were distributed.
2009- This year our slogan was “Get Involved- Together we can make a difference” and 200 t-shirts were distributed with anti-drugs slogan printed on them.
B. Inter-School Speech Competition
An inter-school speech competition was held after the rally and the theme was what roles can school students play in helping to build a drug-free community. 20 students from 10 schools participated in the competition which had an audience of 100 school children and youth.
3. IDUs-AIDS Campaign • World AIDS Day, December 1, 2008
A. IDUs-AIDS Walk Rally
Around 700-800 people participated in the rally and displayed placards and banners with the World AIDS Day slogan.
This year our slogan was “Taking Drugs can lead to HIV/AIDS. Think! and 150 t-shirts with the slogan was distributed mainly to IDUs and risk groups
