10 question basic quiz for my website

Hi, I have made a very simple quiz on goodreads.com. The link is http://bit.ly/wgcwNo but the problem is that the traffic I drive to the quiz has to register with the website before it can take the quiz.
I would like to put exactly the same quiz (as you can see it's very very simple) on my own website. I am looking to keep costs very low so would happily use someone else's template, perhaps slightly modified but I don;t need any fancy reports, just a mark out of ten for how they've done and an option to forward the quiz to friends and family....
