5-Latest P60 (or P45 if you left a job)
6-Any P11D for benefits
7-Self-employment income (if applicable)
8-Total income & expenses
9-Bank interest statements
10-Dividend income
11-Rental income (if you own property)
12-Pension contributions (workplace or personal)
13-Any capital gains (crypto, shares, property, etc.)
14-Child Benefit claim details (for High Income Child Benefit Charge)
15-Any other income (overseas income, job perks, etc.)
