Transcription of short PDF to word

I want someone to transcribe 5 pages of PDF extracts, comprising text and tables (see attached file) to an editable MS word document. I do not need the graphs in the attached document to be re-created, only the text and tables in the attached document. Ultimately, I want to be able to copy and paste the PDF text to another word document and edit that text.

I need someone who is a good transcriber and who can get the job done expeditiously and carefully. I hope to have a 1-2 day turn around.
