Indexing a 200pp book

I need can index created for a 200pp book which is a popular presentation of the beliefs of Islam.
The books is just over 50K words long.
The index will need to include names of people and places, and major subjects.
Ideally the successful applicant will have experience of creating an index, and a sample to show me.
The book is written in English, so English as a first language is preferred.
Some familiarity with islam would also help, but is not essential.
