Application for Professional Membership Upgrade

Hi. I would like to upgrade my professional membership and as part of this process I need to complete an application form which consists of 4 detailed questions about my work experience and achievements including specific examples etc. The application need to be completed in accordance with the institute standards and guidelines. I have already made a start on this but would need some help with elaborating on my answers so that all the boxes are ticked and answers matching the requirements.
