Combo Table Creation

I am needing someone who can create a combo table, which will serve as a binder index.

More specifically, it relates to Canadian Senate and House of Commons bills. Some of them start in the Senate, and end up in the House of Commons. Others, start in the House of Commons, and end in the Senate.

For each bill, there is an associated #, 1st/2nd/3rd reading, debates, major speeches, committee report, report stage and received ascension. I need in essence, a form which I can fill in of the former, with dates and sitting #'s noted. Below is an example of a bill, with the various stages.

Ideally, this form would fit on an 8.5x11 paper (8.5x14 max)
