Wordpress with Member login and My Sql database app

I need a Wordpress site built using a template similar to this:

We need a new site, built from scratch using a standard Wordpress template. The site needs to have a home page designed, with all the templates graphics replaced with a delivery/couriers companies photos and the text will be supplied and replaced.

The wordpress site needs the following pages: Home, Register, Booking, Contacts Us.

The home page needs to have a quote form on it for delivering a parcel worldwide. The form needs from, to, type and size of parcel and weight. Once selected, an automatic quote is displayed. Like: https://wwwapps.ups.com/ctc/request (we want a simpler design, details will be provided)

If the client wishes to go ahead and book a parcel to be delivered, they need to login to the Word press site and complete the booking. The booking is saved to a My Sql database and a pdf label needs to be produced for the customer to attach to the parcel.

The booking is then submitted to UPS with their API: https://www.ups.com/upsdeveloperkit?loc=en_GB

If the client does not have an account on Wordpress, they need to be able to create one. If the client does not have a credit account, they will need to pay for the package delivery by paypal.

We need a separate report for admin only, which displays a list of booking by date range.

I need a fix price quote for the website template modifications, the home page design with a quote form, register page, booking page, Contact us now page and admin area with 1 report for the bookings.

The Wordpress site will be hosted by me.
