Wordpress site development - form & sliders

Currently website exists and is partially complete, easy to add new pages etc and most of the design is finished, images are being designed by graphic designer, mostly needs functionality adding and a few finishing touches.

Need to add form, very similar to attached image, to existing website, sliders need to move up and down, links through to second page which needs to ask whether they want pack sent via email or post, then to third page which will give them a unique reference number, and ask for information on certain companies which once they start to get typed into the box, need to come up as an auto-complete drop down option. Then send through to a thank you page.

System needs to send through information via email, also needs to make sure information is sent through at each of the three stages to account for people not finishing the process, but ensure the same unique code is applied to all three pieces of information sent through.

There are a few extra things need tidying up on the website and adding, the header needs to look a bit nicer and another column needs adding alongside the form/slider. Also some nicer looking header bars over each section.

Logo, images etc are all accounted for, it's mostly development work and tidying up as mentioned above. There is a really short deadline on this and work needs to start immediately, ideally I need the first edit finished by the end of the week, as the website needs to be 100% ready and live by 8th May.

P.S. The website is responsive, based on a theme... the form and any other additions you make also need to be responsive for mobiles etc too.
