Website Designer Needed for new Business Start-Up

I'm looking to work with an incredibly creative designer to build a new website for the following spec

- develop and design page layouts, look and feel to be used throughout the site
- design the navigation scheme
- creation of online application form with data collection
- full build of website including a content management system
- instruction on how to use the CMS
- provision of technical support after the website is online.
