Website design including ability for users to book on a calendar

I am a Domestic Energy Assessor who has a domain and hosting already in place. I am looking for someone to design a website of around 4 pages which will include the ability for a customer to order an EPC , pay for it and schedule it on a calendar. I need access to the calendar as I have to block out hours where I am unavailable. I also need the ability to change the rates that I charge for an EPC although the price is my headline. I then need the meta tags in for South Yorkshire, Chesterfield, Mansfield areas. I have a logo that I can use, I am also accredited by Elmhurst Energy so I can use their logo. Once the booking is placed I would like an email confirmation to go to the client which we can design to say that the booking is provisional and that I will contact them asap to confirm the booking.
