Website admin area built in php

Hi all.

I need a simple website admin area built in php. I need it to be able to add, edit and delete records with around 10 fields. Two or three of the fields will be uploads for images and documents. I need the data to be saved in a mySQL database and accessable through a restful interface which I need you to set up along with the admin area. This is so external sites can query the data so some restful knowledge is a must. I also need a static form page, independent from the admin area but on the same site which will allow customers to populate the form with their details including uploading a file and on submitting the data will be available in the same database so the admin area can view new customer uploads and accept/decline them. Upon
accepting they will be available in the admin area as a current record. The customer can add a location through Google maps so on the form a 'Choose your location' button will be required and when selected the location is saved to the database along with all other form data. The admin area will have an 'All records' tab/page which will show a Google map with all available
records on it using their coordinates.

So to sum up I need a simple admin area with the ability to add, edit, delete and view (via a Google map) all records, a separate form
which customers can submit details for the admin area to approve/decline and Google maps to show all records available. Also with the data tied in to a
restful interface so the data is available to other sites to query.

The admin area also needs to be password protected and it needs to be custom built. We dont want one based on wordpress.
